Our community has mobilized numerous collective impact efforts over time to address urgent needs and put strategies in place to see outcomes improve, but not at the pace or scale necessary for everyone in our community to Thrive.
However, these continued efforts have developed important infrastructure necessary to fuel a movement that has sparked new possibilities as community-led strategies become the cornerstone of solutions and the social fabric and sense of belonging in our community is strengthened.

Thriving Together is a multi-sector, community-driven movement designed to create a sustainable framework for long-term systems change. By strengthening belonging and civic infrastructure, centering community voices, and aligning collaborations and resources, we aim to ensure that all people and places in Winston-Salem and Forsyth County thrive, without exception.

Thriving Together calls us into closer proximity leveraging the 7 Vital Conditions for Wellbeing and Justice.
Stemming out of a national effort during the COVID-19 pandemic and sometimes referred to as the social determinants of health 2.0, the 7 Vital Conditions are the intertwined circumstances that all people and places need all the time to reach their full potential.
While we know that urgent services are necessary, the Vital Conditions Framework calls us to look at the systems that keep us needing those urgent services in the first place and moves us into deeper cross-sector collaboration to multi-solve.

The vital conditions are an evolution, not a replacement, of the social determinants model that has been prevalent since its emergence in the early 2000s.
The World Health Organization (WHO) defines the social determinants of health as “the conditions in which people are born, grow, work, live, and age, and the wider set of forces and systems shaping the conditions of daily life.”
At the heart of Thriving Together lies a core set of values that define not only who we are but also who we are not.
These values shape our actions, guide our decisions, and influence our relationships with each other and our community. They are the essence of our identity and the compass by which we navigate.
